Peterborough Transition-Aged Youth (TAY) Transitional Housing Pilot Project  |  Community Living Peterborough

At the start of this project CL Peterborough hired a consulting firm to research new “best practices" for supported independent living. The organization planned to adapt these innovations in support of their participating community members.Despite regional and international consultations, the research did not reveal any new or noteworthy ideas. The project partners returned to their "tried and true best practice of person-directed planning” which approach has yielded much success. 

As the current group of individuals continues to settle well into their new living situations, pending further funding the organization hopes to replicate the success of this project and support another group of individuals waiting in the wings.


Overview of Outcomes:

  • At present all six individuals are living independently. 
  • Four participants indicated a need for additional supports which CMHA provided.
  • Due to the success of the program, a second TAY case manager added to an existing SIL program, providing capacity to support an additional 6-8 individuals.
  • Support needs decreasing in some cases, for example, one person is currently moving out of a supported environment to greater independence.
  • Participants involved in skill-building activities to assist in maintaining stable mental health and Activities of Daily Living (ADL) skills.
  • Cognitive Behaviour Therapy, Dialectical Behavioural Therapy and Motivational Interviewing skills assist with stable mental health.
  • A cooking group includes meal-planning and food shopping; a certified yoga instructor provides weekly yoga classes to support physical activity, meditation and mindfulness.
  • Entire group is said to be doing well: gaining life skills, connections and new competencies and data sources indicate high participant satisfaction.
