Community Hub Residential Model | Campbellford & Brighton
All six participants in this rural community are now living independently, with personalized supports in place. While project partners collaborated successfully to develop appropriate housing and supports for each individual, at the same time, collaborating also allowed partners to bridge gaps between health and developmental services, resulting in greater efficiency within their own organizations.
Another participant with dual diagnosis experiences complex challenges due to mental health issues. Given such vulnerabilities this individual is prone to becoming caught up in the legal and judicial system. As the project unfolded, things were once again headed in this direction. This time, however, a Quality Assurance Director was available to liaise with the participant and an OPP officer. Fostering awareness and mutual understanding prevented a much more serious crisis from unfolding.

Overview of Outcomes:
- Six adults now residing in a 24-hour community service model no longer require typical residential supports ie. group home/ long-term care.
- Participants are maintaining independence in their own community with supports specific to their needs; able to access and benefit from community-based services such as Occupational Therapy, Physiotherapy, Diabetic Specialists, etc.
- People participating within the HUB model have now have their individual support needs met by using community-based partners and resources, increasing social participation and community engagement for each person supported.
- Multiple partnerships strengthened as a result of the success of the project, bridging gaps between health and developmental services.
